As a member of ZF BKK, you can usually insure your family members free of charge. This means: one premium, many benefits— for you and your loved ones, as long as they do not have their own primary insurance.

Who is also covered by the insurance?

Family insurance covers:

  • Spouses and civil partners (according to the Civil Partnership Act)
  • Your own and adopted children
  • Stepchildren and grandchildren, if you are primarily responsible for their maintenance or if they live with you in the same household
  • Foster children (provided that you are not a professional foster parent)

Requirements for free co-insurance

For spouses and life partners

Your partner can be covered by family insurance if:

  • they are not primarily self-employed
  • there is no statutory insurance obligation
  • their regular total income in 2026 is less than €565.00, or
  • they do not earn more than €603.00 per month from marginal employment

For children

Children are covered free of charge:

  • up to the age of 23, if they are not gainfully employed
  • up to the age of 25 if they are in school, vocational training, or higher education

Periods of voluntary service, military service, or civilian service can extend the co-insurance.

For children with disabilities, family insurance is unlimited in time – provided that the disability existed when entitlement to family insurance still existed.

Note income limits

The following applies to co-insured dependents:

  • For income from self-employment, renting and leasing, capital assets, or a pension:
    maximum €565.00 per month
  • For mini-jobs:
    up to €603.00 per month

If this limit is exceeded, the non-contributory family insurance ends. Please inform us of any changes in income in a timely manner—this helps to avoid interruptions.

Special provisions

If one parent has private health insurance or if the child is a stepchild or grandchild, we will review the eligibility requirements on a case-by-case basis. For example, children can only be covered free of charge if:

  • the income of the privately insured partner in 2025
    does not exceed €6,450.00 per month or
  • your own income is higher than that of the privately insured parent.

Voluntary social year (FSJ)

  • With remuneration: You are compulsorily insured—your employer pays the contributions.
  • Without remuneration: Family insurance remains in place as long as you are under 25 years of age.

Family insurance ending? No problem.

If co-insurance ends—for example, due to taking up employment—the family member can insure themselves directly with ZF BKK. You can find more information on this under "Become a member."

Annual audit – please help us

As a statutory health insurance provider, we are required to review family insurance once a year. When the time comes, you will receive a letter from us with a personal transaction password. You also have the option of accessing the questionnaire without the password, using only your login details for the online office.

Please complete the online questionnaire within 10 days – even if nothing has changed. This will keep your data up to date – and ensure that your family remains fully protected.
